CONSENT FOR ELECTRONIC DISCLOSURES,
AGREEMENTS, AND OTHER RECORDS

Before enrolling for AccessWest® Online Banking Services through Community West
Bank, N.A., you must consent to this agreement. Please read this agreement
carefully and save a copy for your records.
In this Agreement, “you” or “your” refers to the person(s) subscribing to or
using AccessWest Online Banking Service. “We”, “our” or “Bank” refers to
Community West Bank, N.A.
You agree to the following provisions regarding online services received from
Community West Bank, N.A.:

Except for e-statements, which require an additional enrollment, all of the
disclosures, records, AccessWest® Online Banking Services Agreement, and other
information being provided to you may be in an electronic form. Information
provided in an electronic form will not be distributed in paper. You can obtain
a paper copy of an electronic document by printing it yourself or by requesting
that we mail you a paper copy. To request a paper copy, contact us by telephone
at 1-888-831-5295. We may charge you a reasonable service charge for the paper
copies.

Your consent covers each account you have with Community West Bank N.A. now and
in the future where online banking services are available.

For multi-party accounts, consent or withdrawal of consent to receive
electronic disclosures, records, or other information by any authorized party
to the account will be effective for all account holders.

You have the right to withdraw your consent at any time and at no cost to you.
If you withdraw your consent, we will terminate your access to online services,
including bill-pay or other online services. Further, if you withdraw your
consent, your bank statements will no longer be available online. Rather, we
will begin to mail paper copies to you.

If you wish to withdraw your consent, you must contact us via email at AW@communitywestbank.com
or by contacting us at 1-888-831-5295. Withdrawal of your consent will be
effective only after we have a reasonable period of time to process your
withdrawal.

You must notify us about any change in your e-mail address. You may update your
e-mail address by logging onto your online account at
www.communitywestbank.com, accessing your personal online banking system,
clicking on the “Options” link or by contacting us at 1-888-831-5295. If you
fail to give us an updated or corrected email address, we may discontinue your
online banking service until you contact us and provide us with corrected
information.

Below are the hardware and software requirements for access to and retention of
the information being provided to you in electronic form. We will notify you
whenever we change or revise these requirements.

A personal computer or other device capable of accessing the Internet. Your
access to this page verifies that your system/device meets these requirements.

An Internet web browser which is capable of supporting 128-bit SSL encrypted
communications, which requires a minimum web browser version such as Microsoft®
Internet Explorer version 6.0 (available for downloading at
http://www.microsoft.com), Firefox version 3.5 (available for
downloading at http://www.mozilla.com/firefox)
and your system or device must have 128-bit SSL encryption software.

You must have software which permits you to receive and access Portable
Document Format or “PDF” files, such as Adobe Acrobat Reader® (available for
downloading at http://www.adobe.com/products).

To retain a copy of the disclosures, notices, terms and conditions, other
documents, your device must have the ability to download and store or print PDF
files.
